2. Within 14-days of your first Dysport treatment, you must sign up for the Dysport Challenge at www.dysportusa.com , download your Dsyport rebate coupon form and mail the rebate form and itemized receipt. You will receive your $75 rebate whether or not you have a second treatment.
3. Within 15 – 30 days of your Dysport treatment, return to www.dysportusa.com to decide if you Love It or Leave It and download the corresponding rebate form.
4. You must wait at least three months between your first and second treatments.
5. Within 14-days of your second treatment, mail the rebate form and itemized receipt.
